When you book your appointment, please verify your insurance provider with one of our staff members.
Some of our fees cannot be determined before your appointment, but we can provide minimum figures for eye exams and medical visits. We accept:
Care credit is a payment plan designed to help manage high medical costs. 95% of patients are approved on Care Credit and are awarded low interest rates. Speak to our team today to see if you qualify, or visit www.carecredit.com.
Our team can bill Medicare Part B plans for you. However, you must provide us with the most up-to-date information at the time of your visit.
Please be aware that if you have turned your Medicare over to an HMO (also called a Medicare-managed HMO), we cannot bill for services.
We’re also unable to bill if you have turned your Medicare over to Kaiser. We can only bill Medicare that stands alone or Medicare with supplemental insurance.
We’re able to bill your insurance company for our services, but you are financially responsible for the services you receive.
If we don’t receive payment from your health or vision plan within 30 days of your appointment, we’ll contact you to find a solution. Please be aware of what your insurance covers and if we are a part of their insurance network. If you have any questions or concerns, please contact us.
Our office is not a provider for any HMOs. If you have an HMO, you’ll be treated as a private pay patient. We’ll be more than happy to provide you with itemized receipts if you would like to submit them for reimbursement.
Co-payments are due at the time of your visit. If you have an unmet deductible amount, your insurance provider will inform us when we bill them, and you’ll be responsible in full for the amount applied to your deductible. Please note that if we are not a preferred provider of your insurance, reimbursements will be at their “out-of-network” rate.
